Outreach House I & II, located in Ridgewood,
Queens and Brentwood, Long Island,
respectively, provide residential chemical dependency treatment
services for adolescents, 12 to 17 years of age.
The comprehensive program design has been specifically
tailored and enhanced over the years to meet the needs of the
adolescents and families that we serve, incorporating family,
vocational/educational, health, recreational and aftercare services
into the mainstream of programming.
Through a self-help model, the programs utilize positive
peer pressure, along with an organized work structure,
vocational/life skills preparation and individual introspection to
challenge and equip the adolescents with the tools and skills to
assist them in a drug-free lifestyle.
Adolescents come to Outreach House from any number of
sources, including: parents and or other family members, schools and
community-based organizations, court systems and criminal justice
agencies, churches and congregations and other social service
providers. The
adolescents come from every race, culture, family and economic
background and from every part of the five boroughs and Long Island.
The first part of the process is a comprehensive assessment.
A trained assessment work meets with the adolescent and their
parent(s)/guardian(s) to ascertain information about all areas of
the adolescents=
life in order to make a decision as to whether the adolescent
requires and is appropriate for residential treatment.
The information is also utilized in the development of an
individualized treatment plan for the adolescent and their family.
The residential portion of the treatment process actually
consists of three phases: orientation/early treatment, middle
treatment and re-entry. In
order to move from one phase to another, the adolescent must
demonstrate that they have learned a number of skills and behaviors
and are meeting and/or working toward the goals in their individual
treatment plan. As they
progress, they assume greater responsibility and they earn
privileges. When it is
deemed appropriate, the adolescent will begin to go home on passes
and during the reentry phase, they may even begin attending their
home school, while still living at Outreach House.
At the end of the residential portion of
Outreach House, the adolescents participate in a 6-month outpatient
aftercare program which helps them while they transition back to
their home and school environments.
